Віддалені наслідки гістіоцитозу з клітин Лангерганса: органні й гормональні порушення

Анотація

У статті представлено аналіз літератури та власні дослідження авторів з питань діагностики віддалених наслідків гістіо­цитозу з клітин Лангерганса (Langerhans cell histiocytosis — LCH), а також клінічної картини, лікування й довгострокового моніторингу хворих, які перенесли захворювання в дитячому віці.
LCH — гранулематозне захворювання, в основі якого лежить аномальна проліферація і накопичення в органах і тканинах патологічних мієлоїдних клітин, які фенотипічно схожі з клітинами Лангерганса. LCH належить до рідкісних захворювань і його можна характеризувати як мієлоїдну пухлину, яка має запальні властивості. Місцеві та системні запальні ефекти призводять до формування гранульоми й пошкодження тканин у різних органах. Клінічний спектр LCH коливається від одного кісткового ураження (моносистемне ураження; SS-LCH) до ушкодження кількох органів (багатосистемний LCH; МS-LCH). За останні десятиліття різко скоротилася смертність від MS-LCH: при використанні адекватної терапії виживаність пацієнтів становить до 85—90 %. Проте у віддалений період значна частина осіб, які вижили, мають віддалені наслідки захворювання, які впливають на якість їхнього життя. Наявність віддалених наслідків LCH зумовлена передовсім локалізацією патологічного процесу, а ступінь їх тяжкості — деструкцією тканини і утворенням рубців. Вивчення клінічної картини LCH і його віддалених наслідків дає змогу зробити висновок про те, що органні й гормональні порушення (наприклад, центральний нецукровий діабет) часто визначаються вже під час встановлення діагнозу і їх можна розглядати як вияв основного захворювання. Інші патологічні процеси (наприклад, нейродегенеративне ушкод­­ження центральної нервової системи) розвиваються поступово і клінічно маніфестують через місяці, роки й навіть десятиліття. Усе це створює складнощі у проведенні довгострокового рутинного моніторингу для визначення пізніх ефектів захворювання.
